I and my household have provided all information as requested and agree to the conditions set forth by CVRS, Inc. regarding fostering an animal for them. I understand that I will be responsible for the care and transport of the foster for veterinary care, as well as for transporting whenever needed for the purposes of finding a permanent home. I understand that, if I am unable to comply with these stipulations, CVRS will demand that the fostered animal be returned and I agree that, if requested, I will return the animal. I understand that failure to do so can result in legal action. I also understand that CVRS is and will remain the “owner” of this foster animal, until such time as the foster animal is adopted (application for the foster is received, processed, and adoption contract is signed along with payment of the adoption donation). *
